How It Works
We start by meeting with the older adult and his or her family in the comfort of home, where one of our registered nurses will develop a personalized plan of care. The care plan will outline how to help the senior maintain as much independence as possible and stay connected to the community and pursuits that are personally meaningful – all while ensuring safety and comfort.
Some of the many ways we can accomplish this goal include:
- Engaging in enjoyable activities: puzzles, games, music, crafts, etc., per each senior’s personal interests
- Helping the senior stay physically active through physician-approved exercise programs
- Socialization and companionship
- Life enrichment through outings in the community, such as visiting museums, friends and neighbors, the library, etc., as appropriate and desired
- Utilizing creative and effective techniques to redirect inappropriate, difficult, or dangerous behaviors
- Preventing wandering and maintaining a safe home environment
- Providing personal care assistance such as with bathing/showering, getting dressed, preparing meals, etc.
