An in-home consultation
An in-home consultation
First, we visit with the older adult and his/her family members, as appropriate, at home. This complimentary consultation gives us the opportunity to get to know the older adult better, and to begin to piece together the services that would best fit in his or her particular situation.
A customized care plan
A customized care plan
Our registered nurse will then create a plan of care and personalize it for the older adult. Each care plan outlines the services we recommend, including when in-home care visit will occur based on the desired schedule and routine. The care plan will then be regularly assessed and adjusted as needed.

We carefully select a Care Provider who has the experience needed and shares similar interests, likes, dislikes and personality type to ensure a good connection.

Care & Support
The care provider, care coordinator, and the nurse review the care plan together at the start of service. The care coordinator personally introduces the care provider for the first visit. Ongoing care, assistance, and communication are available for our clients and their families with a designated care team, including a care coordinator, along with direct access to the family portal.
